High-Yield Cardiology MCQs for Gulf Exams

Cardiology is the single highest-weighted clinical domain across every Gulf licensing exam for General Practitioners. Master these topics and you'll bank a large chunk of your total score.

Topics that appear on every exam

  1. Acute coronary syndromes — STEMI vs NSTEMI vs unstable angina, MONA-B protocol, thrombolysis contraindications.
  2. Hypertension — staging, first-line agents by comorbidity, resistant HTN workup.
  3. Heart failure — NYHA class, HFrEF vs HFpEF, guideline-directed medical therapy.
  4. Arrhythmias — AF rate vs rhythm control, CHA₂DS₂-VASc scoring, WPW recognition.
  5. ECG interpretation — MI localisation, bundle branch blocks, hyperkalaemia changes.
  6. Valvular disease — aortic stenosis murmur, mitral regurgitation, endocarditis prophylaxis.

Worked example

A 62-year-old man presents with 45 minutes of central crushing chest pain radiating to his left arm. ECG shows 2 mm ST elevation in leads II, III and aVF. Which artery is most likely occluded?

A. Left anterior descending
B. Left circumflex
C. Right coronary artery ✓
D. Left main

Inferior STEMI (II, III, aVF) is right-coronary-artery territory in ~80% of patients. Always check right-sided leads (V4R) for right-ventricular involvement, which changes fluid management.
